Is healthfirst essential plan Medicaid?
The Essential Plan is a New York health insurance program for lower-income state residents who don’t qualify for Medicaid or the Child Health Plus program. There are four Essential Plan options, each based on household income level (see below).

Essential Plan 2 – Individuals with income greater than 138% and less than or equal to 150% of the FPL. Essential Plan 3 – Individuals with income equal to or greater than 100% and less than or equal to 138% of the FPL and not eligible for Medicaid due to immigration status.

You need a referral from your Primary Care Provider (PCP) to see a specialist. Only your PCP can refer you to another doctor. Call the PCP listed on your Medicaid or Health Choice ID card.

You may qualify for Essential Plan if: You are a NYS resident. Your monthly income in 2020 is below $2,127 for a single person or $4,367 for a family of four. You are a US/Naturalized Citizen or an immigrant present with the knowledge and consent of the US immigration service.
